Pet Transportation Service Agreement and Liability Waiver

Effective Date: August 24, 2026

This Pet Transportation Service Agreement (“Agreement”) is entered into between Chicago Animal Ambulance, Inc. (“Chicago Animal Ambulance”) and the undersigned pet owner or authorized person (“Client”) concerning transportation of the animal identified below (“Pet”).

5. Owner's Representations

Client represents that, to the best of Client's knowledge, all information provided concerning the Pet is accurate.

Client agrees to disclose any known condition or circumstance that could reasonably affect safe transportation, including, when applicable:

  • Aggressive or fearful behavior;

  • Bite or scratch history;

  • Escape behavior;

  • Medical conditions;

  • Seizure history;

  • Mobility limitations;

  • Medications;

  • Contagious illness;

  • Special handling requirements;

  • Restrictions concerning contact with other animals; and

  • Any other condition reasonably relevant to transportation safety.

Client represents and warrants that Client is the owner of the Pet or is authorized by the owner to arrange transportation and provide the instructions contained in this Agreement.

Client is responsible for ensuring that Chicago Animal Ambulance receives complete and accurate information necessary to safely perform the transportation service.

10. Emergencies

Client authorizes Chicago Animal Ambulance to take reasonable steps consistent with the separate Emergency Transportation and Veterinary Care Authorization, when applicable.

Client understands that emergency veterinary care may involve costs for which the owner or responsible person remains responsible unless Chicago Animal Ambulance has expressly agreed otherwise in writing.

Client acknowledges that transportation personnel are not substitutes for licensed veterinarians unless expressly acting in a legally authorized professional capacity.

Client must disclose known medical conditions that could affect transportation.

​Chicago Animal Ambulance is not responsible for deterioration of an undisclosed pre-existing condition except to the extent liability cannot legally be excluded or is caused by conduct for which Chicago Animal Ambulance is legally responsible.

9. Medical Conditions

Client agrees to provide or permit use of appropriate collars, harnesses, leashes, carriers, crates, or other equipment reasonably necessary for safe transportation when requested by Chicago Animal Ambulance.

Chicago Animal Ambulance may refuse transportation when the Pet cannot reasonably be handled or secured safely.

8. Pet Equipment and Safe Handling

Client understands that transportation may involve risks, including but not limited to:

  • Anxiety, fear, or stress;

  • Motion sickness;

  • Dehydration;

  • Escape;

  • Illness;

  • Aggravation of an existing medical condition;

  • Behavioral incidents;

  • Injury;

  • Exposure to environmental conditions;

  • Traffic or transportation delays;

  • Vehicle-related risks;

  • Contact with other animals; and

  • Unforeseen circumstances or emergencies.

Client acknowledges these risks and agrees that Chicago Animal Ambulance cannot guarantee that transportation will be free from all risks.

7. Risks of Transportation

6. Transportation Services

Chicago Animal Ambulance will use reasonable care in providing transportation services consistent with its policies, available resources, and applicable law.

Transportation may involve ordinary risks associated with moving an animal outside its home environment.

Chicago Animal Ambulance may decline, postpone, modify, or discontinue transportation when reasonably necessary to protect the Pet, personnel, other animals, members of the public, or property.

15. Fees and Payments

Any applicable transportation fee, deposit, reimbursement, or other payment arrangement will be communicated separately.

Chicago Animal Ambulance is a for-profit Illinois corporation and is not exempt from federal income tax under Section 501(c)(3) of the Internal Revenue Code. Payment of a transportation fee is compensation for services rendered, not a charitable contribution, and is not tax-deductible. Payment of a fee does not create a guarantee of service beyond what is expressly stated in this Agreement.

Client acknowledges that Chicago Animal Ambulance relies upon information provided by Client when determining whether transportation can safely be performed.

Client agrees to notify Chicago Animal Ambulance promptly if relevant information changes before transportation.

14. Client Responsibility for Accurate Information

Client should remove valuable personal property from carriers, vehicles, crates, or other belongings provided with the Pet unless the property is reasonably necessary for the Pet's care.

13. Property

To the maximum extent permitted by applicable law, Client acknowledges and assumes the ordinary and reasonably foreseeable risks associated with transporting an animal.

Client releases Chicago Animal Ambulance and its directors, officers, employees, contractors, agents, and representatives from claims arising solely from inherent or ordinary risks of animal transportation, except to the extent such release is prohibited by applicable law or the claim results from conduct for which liability cannot lawfully be released.

Nothing in this Agreement is intended to release liability for gross negligence, willful or wanton misconduct, intentional misconduct, or any other liability that applicable law prohibits a party from waiving.

12. Release and Assumption of Risk

11. Delays, Weather, Traffic, and Unforeseen Circumstances

Chicago Animal Ambulance cannot guarantee an exact pickup or arrival time when circumstances outside its reasonable control affect transportation.

These circumstances may include traffic, severe weather, road closures, vehicle issues, emergencies involving another animal, government restrictions, unsafe conditions, or other unforeseen circumstances.

Chicago Animal Ambulance will make reasonable efforts to communicate material delays when practicable.

By signing this Agreement or providing legally valid electronic consent, Client acknowledges that Client has:

  • Read this Agreement;

  • Had an opportunity to ask questions;

  • Provided truthful and complete information;

  • Disclosed known safety and medical concerns;

  • Understood the inherent risks of animal transportation; and

  • Agreed to the applicable terms.

18. Client Acknowledgment

Chicago Animal Ambulance does not guarantee a particular medical, behavioral, transportation, or other outcome.

17. No Guarantee of Outcome

16. Cancellation

Chicago Animal Ambulance may establish cancellation, scheduling, and refund terms appropriate to the service requested. Any applicable terms will be communicated before or at the time of booking.
